Enrollment Info

Lakeside Academy invites you to take a tour of our facility.  Appointments are not necessary, but we suggest that you visit during peak activity hours of 9am-12pm.  Enrollment applications are available at our front desk.

To enroll you will need:

	Tour/interview with our staff and your child
	Completed enrollment application
	Shot record (must be up-to-date)
	$60.00 registration fee
	Physician's Report within 30 days after enrollment
